Welcome to Chemed’s Hypertension Program!
CHEMED’s Case Management Department has recently started a new program to help patients manage their blood pressure. The program will run from 2021-2023 and is open to CHEMED patients.
We will be providing free blood pressure monitors for eligible patients to monitor their own blood pressure at home. Checking your blood pressure can help you and your doctor figure out if you need medications or that your medications are working or need to be changed. As a participant, you will be assigned a personal case management nurse who will be checking in with you to help you reach your blood pressure control goals.
